Additionally\, we offer generous PTO accruals which includes paid sabbatical leave. \n  \nAbout the Role: The Certified Peer Specialist- MAT provides integral substance use recovery supports for patients enrolled in Project HOME Health Services Medication for Opioid Use Disorder medication assisted treatment (MAT) services. Grounded in a person-centered and trauma-informed approach\, supports patients in identifying needs and overcoming barriers to recovery including but not limited to integration of physical and behavioral health needs\, social support\, occupational and educational challenges\, housing\, and income stability \nPay Rate is $19.20/hourly\nLocation: Stephen Klein Wellness Center at 2144 Cecil B. Moore Avenue\, Philadelphia\, PA 19121.\nHours: Monday – Friday 8:30 – 4:30 pm \nEssential Duties and Responsibilities: \n\nCollaborate with Project HOME Healthcare Services staff and patients to support patient health and well-being\n\n\nSupport patients in development of individual recovery\, relapse prevention\, and harm reduction plans as appropriate\n\n\nDevelop and encourage patient attainment of measurable action steps related to individual recovery goals\n\n\nProvide recovery education and support along the continuum from pre-treatment engagement to recovery engagement\, stabilization\, and maintenance\n\n\nReport care management activities and complete documentation in a timely manner including progress notes\, evidence-based screenings & assessments\, and program operational tools to support with quality improvement & data collection\n\n\nRegularly participate in MAT program and care coordination meetings\n\n\nAccompany patients/clients to critical health care or social service appointments\n\n\nPromote the utilization of resources within the community\n\n\nTeach and support acquisition and utilization of skills needed for recovery\n\n\nHelp to facilitate the development of a sense of wellness and self-worth\n\n\nEngage in continuing professional development and training as identified and agreed upon with supervisor\n\n\nOther duties as assigned\n\nWhat You’ll Bring: \n\nHigh school graduate or GED. REQUIRED\n\n\nCertified Peer Specialist or Certified Recovery Specialist\, at least 1-2 years professional experience in behavioral health treatment/direct service settings working with recovering persons with experiences of homelessness\, mental health concerns\, substance use/dependence\, and trauma. REQUIRED\n\n\nLived experience identifying as a person in recovery.\n\n\nExperience in providing substance use treatment using harm reduction\, trauma-informed\, and anti-racist frameworks.\n\n\nDemonstrate understanding and respect for each individual’s unique path to recovery.\n\nWhat Qualifications or Experience You May Bring: \n\nKnowledge of Philadelphia behavioral health\, substance use/dependence\, and housing systems\, services\, and resources.\n\n\nCompletion of Certified Peer Specialist or Certified Recovery Specialist curriculum with current certification or in good standing for certification renewal.\n\n\nStrong crisis intervention\, de-escalation skills.\n\n\n\n\nCultural humility and experience serving diverse\, under-resourced communities\n\nDeep commitment to health equity\, inclusion\, and trauma-informed care\n\n\nExperience working in integrated primary care and behavioral health settings or other interdisciplinary settings\n\n\nExperience facilitating/co-facilitating recovery-focused groups\n\n\nBilingual (Spanish/English) appreciated\n\n\nValid Pa driver’s license\n\n\n\n\n\n  \n\n\n\nCore Skills:\n\n\nStrong organizational skills.\n\n\n\n\nVery strong verbal and written communication\, organizational\, and interpersonal skills\n\n\n\n\nTeam experience and collaborative approach to service delivery\n\n\n\n\nStrong computer literacy and ability to learn eClinical Works Electronic Health Record system\n\n\n\n\n\n  \n\nPhysical Requirements:\nStanding and walking most of the day; light lifting\nAbout Project HOME: The mission of Project HOME is to empower adults\, children\, and families to break the cycle of homelessness and poverty.
